Regular Hours
Forward Family Therapy is open six days per week for your convenience, including Saturday's. The office is generally open beginning at 8 a.m. and we offer evening hours up to 8 p.m. depending on the therapist's individual schedules.

$175 per 45-50-minute session for Pamela Boaz, LCSW
$130 per 45-50 minute session for April Betty, LPC
$150 per 45-50 minute session for Brandon Buenavidez, LPC
$150 per 45-50 minute session for Emily Cunningham, LCSW
$150 per 45-50 minute session for Crystal Huynh, LPC
$150 per 45-50 minute session for Kate Leak, LCSW - Kate also offers EMDR Intensives at $170 per hour.
$150 per 45-50 minute session for Anastasia Mitchell, LPC
$120 per 45-50 minute session for Skye Wyrick, LCSW
Insurance
Pamela Boaz, LCSW is currently a credentialed provider for Lyra and Kaiser insurance companies. However, many clients choose to see Pamela as an Out of Network Provider. We are glad to assist you with this process if we do not take your insurance. Crime Victim Compensation is gladly accepted as are clients choosing to pay for treatment themselves.
Brandon Buenavidez, LPC is currently a credentialed provider for Anthem BCBS and also accepts both cash paying clients as well as client using Victim Compensation benefits. Clients may also choose to use their out of network benefits, which we will gladly assist.
Emily Cunningham, LCSW is currently a credentialed provider with Anthem/Blue Cross/Blue Shield and Kaiser and accepts both cash paying clients as well as clients using Victim Compensation benefits. Clients may also choose to use their out of network benefits, which we will gladly assist.
April Betty, LPC is currently a credentialed provider for Lyra and Kaiser insurance companies. She also accepts cash paying clients as well as Victim Compensation benefits. Clients may choose to use their out of network benefits, which we will gladly assist.
Crystal Huynh, LPC is currently a credentialed provider for Anthem Blue Cross/Blue Shield, and Kaiser. She also accepts cash paying clients as well as Victim Compensation benefits. Clients may choose to use their out of network benefits, which we will gladly assist.
Kate Leak, LCSW is currently credentialed with Kaiser and is awaiting credentialing with Aetna. She also accepts cash paying clients as well as clients using Victim Compensation benefits. Clients may choose to use their out of network benefits, which we will gladly assist.
Anastasia Mitchell, LPC is currently accepting clients through Kaiser, the Crime Victim Compensation program, clients who wish to use their out of network benefits as well as cash paying clients.
Sarah Russ, LCSW is currently accepting clients through the Victim Compensation program, clients who wish to use their out of network benefits as well as cash paying clients.
Skye Wyrick, LCSW is currently credentialed with Kaiser, Aetna and Anthem BCBS/Beacon. She accepts clients using Victim Compensation benefits. Clients may also choose to use their out of network benefits, which we will gladly assist.
Payment
Payment is due at the time of the session. If you are using insurance, only your co-pay will be due at the time of service. If you have been approved for Victim Compensation benefits, no payment is due. Cash, check and credit cards are all accepted forms of payment.
Cancellation Policy
Because we have reserved your appointment time for you, you will be expected to pay for your session unless you provide 24 hour cancellation notice, except in unforeseen emergency situations.
*Please note that the cancellation policy for EMDR Intensives is different and you must provide 21 days notice to avoid being charged a fee.
